 Literature & Materials Order Form

 When ordering, please provide ID numbers and be sure to
include the format designation. For example, to order a Braille
copy of the 1968 Banquet Speech, "Blindness - Milestones and
Millstones" indicate LAA68B. Reprints are not always available in
all formats. The Braille Monitor from which a reprint is taken is
included on the form for those who need the information in
additional formats. Those ID Numbers begin with the letter M.

       Literature and Materials are provided by the Materials Center
of the National Federation of the Blind as a service to blind
individuals. Since this is a non-profit operation and since it is
desirable to keep prices low, orders should be sent by mail on this
order form whenever possible. If a rush is necessary, we will
accept orders by telephone. Please allow four to six weeks for
processing and shipping of literature and materials orders.

       Publications are provided free of charge, unless a price is
noted. Back issues of magazines 13 months or older are $2.00,
regardless of format. We require prepayment by check, money order,
purchase order, or credit card (Discover, MasterCard, or Visa).
Individuals receive a discount. The discounted prices are listed in
parentheses. Prices are subject to revision and may be changed
without notice.
